An opportunity to build rapport to identify strengths, goals and expectations. We identify areas of needed support with family, caregivers, and current service providers​.

Meetings take place in the home, community or online. Our primary goal is to build rapport, confidence, self-awareness.


Sessions cover a wide range of life skills essential for independence. They include self-care, vocational and educational goals, transportation, social skills and more. We take a practical, hands-on approach teaching skills that can be immediately applied in daily life.

 
We assist clients with access to community supports. This can come in the form of social groups, public transportation training, volunteer experiences, career supports and other accommodations. We use a collaborative approach building a supportive team around each client.
